To Schedule An Appointment, Please Follow These Steps:
1. Using Insurance?
If I'm credentialed (in-network) with your insurance and you plan to use it, you must first create an account through Headway, which manages all insurance billing. You do this by clicking the My Headway Profile button.
2. Request an Appointment
Once your Headway account is set up and your insurance has been verified (or if you’re cash/self-pay), return to this website and click “Book Appointment.” You’ll select a time from the calendar, enter your contact information, and add a valid debit or credit card to complete your request.
3. Complete Intake Forms
Once you’ve completed the insurance steps (if applicable) and requested your preferred appointment date and time through the “Book Appointment” process, you’ll receive an email from Hilbert’s Whole Health Psychiatry LLC titled “Please fill out this form.” To confirm and secure your appointment, please complete the electronic intake paperwork at least 48 hours in advance.

4. Download the Spruce Health App
Spruce Health is a free, secure app used for all communication with your provider. If you haven’t received an invitation yet, a link will be sent via email and text. Please accept the invite, download the app, and TURN ON notifications. You can also scan the QR code on the “Services” page of this website to go directly to Hilbert’s Whole Health Psychiatry on Spruce.

We are in the process of becoming credentialed with Blue Cross Blue Shield of Nebraska, Blue Cross Blue Shield of Massachusetts for Oregon residents, and Independence Blue Cross (Virtual National Network) for Oregon residents.
Out-of-Network Coverage
If I am not in-network with your insurance plan or am still in the process of becoming credentialed, you have the option to pay out of pocket at the time of service. I will provide a detailed super bill that you can submit to your insurance company for reimbursement. Reimbursement eligibility and amounts vary based on your plan’s out-of-network mental health benefits, including applicable deductibles or coverage limits. I strongly recommend contacting your insurance provider in advance to confirm your out-of-network mental health coverage and understand the reimbursement process.

Cash/Self-Pay Monthly ADHD Plan - $110
A monthly cash/self-pay ADHD stimulant medication plan may be available for stable individuals with uncomplicated ADHD who follow treatment guidelines and attend regular follow-up appointments. Eligibility is determined at my clinical discretion based on your health history, treatment response, and overall needs. To be considered for this plan, a new patient intake, a comprehensive ADHD evaluation conducted by me, and completion of required consent forms are necessary. Baseline labs (within 6–12 months) and an ECG (within 24 months) are usually required to ensure safe prescribing.

Opting Out of Insurance
If I’m in-network with your insurance but you prefer to pay privately, you’ll need to complete a simple insurance opt-out form, which I’m happy to provide upon request. This acknowledges your choice not to use insurance for services. Participation in the self-pay ADHD subscription plan is subject to clinical appropriateness and provider approval.

• Telehealth or In-Person Appointment – 90 minutes – $450
Please note: Intake paperwork and consent forms must be completed at least 48 hours before your appointment, or it may be subject to cancellation. Payment (copay or full fee) is due in full before the session begins.

Follow-Up Appointments (Established Patients)
• Telehealth or In-Person Appointment – 30 minutes – $275
