1
In-Network Insurance Coverage
If you plan to use insurance, please first verify that I’m in-network with your plan.
To do this, click the "My Headway Profile" button below, which will take you to Jessica Hilbert's Headway profile page. From there, click on the Add Insurance button and enter your insurance card information—you’ll receive a prompt response confirming whether I’m credentialed with your plan. Currently, Headway manages all insurance billing.
Once verified, return here and click the "Book Appointment" button to request a date and time for your visit.
2
Out-Of-Network (OON)
Coverage
If I’m currently out of network with your insurance and you would still like to schedule an appointment, you would pay for the service upfront, and a superbill would be provided, which you would then submit to your insurance for possible reimbursement.
Reimbursement depends on your plan’s out-of-network mental health benefits, including deductibles and coverage limits, so it’s best to contact your insurance provider in advance to confirm your eligibility and reimbursement details.
3
Opting Out of Using Insurance
If I’m in-network with your insurance but you prefer to pay privately (for various reasons, including high copay), you’ll need to complete an insurance opt-out form, which will be provided in your intake paperwork. This form confirms your choice to self-pay for services.
To request an appointment, click the "Book Appointment" button on this page.

APPOINTMENTS & FEES
UPDATED FEES - AS OF 11/24/25
To make care more accessible amid rising healthcare costs and the increasing number of people opting out of insurance, I’ve restructured and lowered my fees effective 11/24/2025 (reflected below), while maintaining the same high level of individualized, evidence-based care.
Initial Integrative Psychiatric Evaluation (New Patients) • Telehealth or In-Office
*90 minutes – $300
PLEASE NOTE: Intake paperwork and consent forms must be completed at least 72 hours before your appointment; otherwise, it may be subject to cancellation.

ESSENTIAL: While intake forms may seem inconvenient, completing them THOROUGHLY and with detail—including sending recent and past lab results (via Spruce app), current medications & supplements (if any), complete health history & past treatments, current health problems, life stressors, symptoms, etc., is vital to maximizing your time. Doing so enables me to conduct an in-depth pre-visit review and analysis, spending time preparing a detailed summary for our first visit. Such preparation allows the appointment to identify specific areas of concern, your needs and goals, reviewing potential underlying contributing factors, and collaborative treatment planning—rather than simply reiterating background information, we can build a foundation, explore patterns, and gain insight into the whole picture, ensuring you get the most from our time together.
Follow-Up Appointments (Established Patients) • Telehealth or In-Office
*30 minutes – $150
PLEASE NOTE: Between appointments, I dedicate additional time to reviewing any new lab results (if applicable), relevant evidence-based research, and refining treatment plans to ensure that care remains comprehensive, individualized, and effective.
Sliding Scale
Based on verified household income and current circumstances, a sliding-scale fee option may be available for individuals without health insurance and experiencing financial hardship. Rates are reviewed periodically to ensure fairness and sustainability. Even with adjusted fees, patients are expected to remain active participants in their care, follow treatment plans that include recommended lifestyle changes, which may involve adjustments to their diet and support with supplements. These modifications can add to overall costs for some, but I strive to be as mindful as possible when creating sustainable and cost-effective treatment plans.
INTAKE & CONSENT FORMS
Complete & Submit All Forms
-
You’ve completed the insurance steps (if applicable) and requested your preferred appointment date and time through the “Book Appointment” button.
-
Next, you should receive an email from Hilbert’s Whole Health Psychiatry LLC, titled “Please fill out this form.” To confirm and secure your appointment, please complete the electronic intake paperwork at least 72 hours before your scheduled visit.
-
If your insurance is in-network and you plan to use it, you will receive consent forms from Headway that must be completed as well.

REMINDER: The intake and consent forms may seem like a one-time inconvenience, but they’re essential for providing thorough, personalized care. Completing them thoughtfully allows me to review your history in detail before we meet.
